Every page is formatted for A4 paper and designed to print cleanly single-sided — including the complaint letter, which is its own separate page ready to be signed.
How to fill in your name and address on the complaint letter
The complaint letter uses placeholders like [Your full name], [Your address], [Your phone number] and [Your email address] where your personal details go. These aren't clickable form fields — the PDF is a document, not a form — so there are two simple ways to complete it:
The easiest option. Print the letter, then handwrite your details in place of each bracketed placeholder. This is completely acceptable for lodging with the Fair Work Ombudsman.
Open the PDF in a free online tool like Adobe Acrobat's "Fill & Sign", Smallpdf, or PDFescape, click on each bracketed placeholder, and type your details over it. Save the edited file, then print or attach it to an email.
Once your details are filled in, sign and date the letter. You can lodge it directly at fairwork.gov.au/complaints, attach it to an email complaint, or bring a printed copy if you speak to the Fair Work Ombudsman by phone (13 13 94).
